SUMMARY
This person is responsible for completing conversational interviewing for the SAMHSA grant and following up with patients/clients to see if the educational information shared has impacted their behavior. (Reduced harm). This position is responsible for providing training and education about the risks of substance misuse and HIV. Provides needed linkages to service provision for individuals with HIV in Orange County. The focus is to provide services to Black Heterosexuals, Men who have Sex with Men (MSM), Transgender Women, and Transgender Men throughout Orange County who are at high risk for HIV and substance use disorders.
ESSENTIAL DUTIES AND RESPONSIBILITIES include the following:
? Provide training and education about the risks of substance misuse and HIV.
? Provides needed linkages to service provision for individuals with HIV in Orange County.
? Provides services to Black Heterosexuals, Men who have Sex with Men (MSM), Transgender Women, and Transgender Men throughout Orange County who are at high risk for HIV and substance use disorders.
? Provide HIV education to individuals identified as high risk
? Provide HIV and Hepatitis screening for all individuals referred to the program
? Link high-risk individuals to PrEP services
? Link individuals eligible to Ryan White Services
? Link individuals not eligible for Ryan White services to Qualified Healthcare Centers to receive medical care on a sliding fee schedule
? Partner with AETC to offer at least two annual training to substance use and medical providers in the area on the importance of screening for HIV and care for those infected with HIV and (Assist the County in developing a media campaign to educate the community on substance use, its relationship to HIV infections, and available services
? Manage a diverse caseload of adults and accept warm handoffs from clinical providers for intake, assessment, case management, referrals, and linkages to care. Caseload requirements are determined by site and program
? Follow up with the patients that were referred regarding access and availability of treatment resources, and provide additional referrals as requested
? Secure all necessary releases from clients/patients to be able to report all data collected on the contacts, referral services, and follow-up services on the County's approved electronic database management system
? Maintains comprehensive community resources and networks to assist patients and families as they transition back to the community
? Maintains professional competency by participating in continuing education and appropriate internal and external learning experiences
? Participates in the development of programs to support patients and families diagnosed with HIV and substance abuse diagnosis
? Effectively communicates with patients, families, staff, and community leaders to further development of comprehensive support for the patient. Ensures effective communication of pertinent medical information is provided to the patient
? Facilitate referrals for Clinically indicated services outside the primary care clinic, e.g., Social Services such as housing assistance, vocational rehabilitation, mental health specialty care, and substance abuse treatment
? Implement Evidence-based interventions such as motivational interviewing Screening, Brief Intervention, Referral to treatment, and Trauma Informed care
OTHER RESPONSIBILITIES
? Performs other work-related duties and special projects as assigned.

? Has no direct supervisory responsibility

EDUCATION and EXPERIENCE
Bachelor?s degree in business, healthcare administration, or related field and over five (5) years of experience in healthcare business operations and strategic planning OR equivalent combination of education and experience. Solid management experience in healthcare, including personnel management, overseeing and managing contracts and services, and promoting and enhancing overall operations.
CERTIFICATES, LICENSES, and REGISTRATIONS
A valid Florida Driver?s License is required.
